Services Prerequisites

 Services listed below are provided free of charge to qualifying residents. Federal, State and Local funding sources require that Legal Hospice of Texas have the following information verifications on file to provide you with legal assistance. First-time clients who are HIV+ must have a referral from one of the Dallas County Services

Required Verification Information:

  • Current Photo I.D.
  • Proof of residency (must live in one of these counties: Dallas, Collin, Denton, Henderson, Hunt, Rockwall, Kaufman, Ellis, Cooke, Navarro, Grayson, Hood, Parker, Johnson, Tarrant and Fannin)
  • Physician's Letter of Diagnosis
  • Verification of your disability - (if applicable) e.g. determination letter from Social Security or letter from private disability insurance company
  • Proof of Income - most recent pay stub, Social Security payment, etc.
  • Copy of Referral (if applicable) from Intake Site listed below
